I can't help feeling when ever I see cases of this nature a slight niggling doubt as to whether surgery is the best course of action.
Any thoughts anyone?

Yeah, we now have more options and choices, and so the ethical considerations become more complicated. However, we are not understanding all of these options in a more complicated way, but merely following our selfish desires.
That is an over simplification, and there is the very real issue of hormonal imbalance and other medical conditions to consider; things that we are still barely aware of, but have been in denial of, for most of Human history.
